Geriatric Mental Health
Community Teams

The Geriatric Mental Health Community Teams aim to alleviate the effects of serious mental illness by providing specialized outreach consultation and treatment services.  The service is delivered in a caring, sensitive and respectful manner to enhance the quality of life of seniors.

This is achieved through a collaborative mental health care model involing referring/primary care physicians, psychiatrists and community agencies.

The Service is for adults in Lanark, Leeds & Grenville who are at least 65 years of age and are living with a serious mental illness such as depression or dementia with related behavioural problems that make it difficult for them to manage at home or in a community setting.


We also offer support services for families and caregivers.
