Assertive Community Treatment Teams
Eastern Ontario Team
The Eastern Ontario Assertive Community Treatment Team (ACTT) is a community-based outpatient treatment program of the Brockville Mental Health Centre
Specifically, the Team provides services to residents of Stormont, Dundas, Glengarry, Leeds, Grenville, Lanark, Prescott and Russell counties who have a serious mental illness and require ongoing intensive treatment.
More specifically, the Team accepts patients who are at least 16 years old and priority is given to frequent users of mental health services who have significant functional impairment and continuous high service needs due to a serious mental illness.
Program Goals
The ACTT Program provides intensive treatment, support and rehabilitation services to patients in the community, in order to:
- Prevent unnecessary admissions to hospital (or reduce the length of time in hospital if required);
- Help patients improve their quality of life in the community; and
- Help patients reach their optimum level of independent functioning.
Services Provided
The variety of services provided by the ACT Team includes:
- Medication monitoring, supervision and education;
- Regular contact in the patient's own environment to provide support, assess their health mental health status, and modify the treatment plan as necessary;
- Collaboration with families and agencies;
- Housing assistance;
- Recreational support;
- Life skills teaching;
- Continuity of care with no limit on length of time with the service;
- Day, evening and eventual weekend access;
- Bilingual staff;
- Vocational counseling and support;
- Behavioral assessment and treatment; and
- Emergency after-hours service.
Referrals for Admission to the Program
Referrals to the program are accepted from the patient themselves, or from mental health-care facilities, family, community providers and family physicians.
Referrals made in Prescott and Russell can be made by calling 613.764.5464. In Stormont, Dundas and Glengarry, referrals can be made by calling 613.933.7184, while referrals for residents of Leeds, Grenville and Lanark should be directed to 613.498.1492.
About the ACT Team
The Assertive Community Treatment Team is composed of a team leader along with a variety of professionals from several health disciplines, including:
- Psychiatrists;
- Nurses;
- Social workers;
- Occupational therapists;
- Recreation therapists;
- Behavioral therapists;
- Vocational therapists;
- Addiction specialists;
- Community mental health workers; and
- Peer support workers.
Each patient receives services from the entire team, based on their individualized treatment plan. The Team works together to emphasize patient choice and implement an assertive approach featuring individualized services, outreach and relationship-building.
