Assertive Community Treatment Teams
Assertive Community Treatment Teams are part of a community-based outpatient treatment program coordinated in partnership with the Brockville Mental Health Centre.
The ACTT Program provides intensive treatment, support and rehabilitation services to patients in the community who have significant functional impairment and continuous high service needs due to a serious mental illness in order to:
- Prevent unnecessary admissions to hospital (or reduce the length of time in hospital if required);
- Help patients improve their quality of life in the community; and
- Help patients reach their optimum level of independent functioning.
ACT Teams are located in Brockville (serving Leeds-Grenville, Lanark counties), Casselman (serving Stormont, Dundas and Glengarry counties), and Cornwall (serving Prescott-Russell counties).
